re-writing my review on these. there is a cloth ring that connects the Anatote to your caribeaner, that clicks & connects to one of your belt loops. this is the second one I've had to buy and both of them have come undone. the second one I bought didn't last long at all. haven't seen anything else I can use, but still, it..is...disappointing that it does not hold up.

Anatote
The Anatote is very sturdy without being bulky. It has loops and a clip for attaching to a belt or waistband, plus a carabiner for attaching to a belt loop, backpack, purse strap, etc. I was especially pleased to discover that there are 2 sets of elastic loops, enabling me to attach it to my belt horizontally or vertically depending on my outfit and planned activity. The Anatote is also very plain--black with some red trim--which makes it less obvious that I'm carrying an Epi-Pen. My friends, family, and co-workers all know what it is, but to the casual observer it might be a cell phone or mp3 player. It's good to know I have the emergency medication I might need without advertising to the world, 'hey, look, I have allergies!'

Great case for Epipen and Benadryl on the go!
I bought three of these Ana-Totes 3 years ago (directly from the company) after my son's anaphylactic reaction to nuts. We keep one in my purse, one in my husband's backpack, and one we got in anticipation of him carrying his own Epipen, but that hasn't happened yet except at daycamp, where they clipped it onto his beltloop. I use the carabiner hook whenever I take the Ana-Tote out of my purse if I'm carrying a larger bag, like to a game or event, and I hook it on the outside of that bag. Grandma, my husband, everyone can see it and know where to find it in an emergency. The pouch holds one Epipen and two Benadryl prefilled spoons, which I put it first, with the Epipen on top. The loop that the carabiner hooks onto is still holding strong after three years. This is the perfect size for my small purse and it's bright color lets everyone know where it is. For school and daycare, they have a kit containing Benadryl and two Epipens for my son, but for on-the-go, when we will be close to an ER, this is perfect. I highly recommend this little pouch!

Item not correctly pictured
I'm sure this item will still hold the eip-pen just as well, however the ONLY reason I bought it is because it is pictured having a keyring type hook so I could use it as a keychain and always have it with since I don't carry a purse. Instead there is a cheap D-ring type hook on it that I wouldn't trust to keep the epi pen attached to anything. Not happy with this purchase at all.
