Q: Is there a fly in these briefs?

Yes there are my husband only uses these briefs for many years he’s pleased with either them

Q: the picture of the underwear on the model does not jive with the picture of the underwear by itself

I received this model with a grey tag on the waist, instead of red tag as pictured. The newer grey tag model (made in Vietnam) irritates and has rougher materials compared to the original red tag model (made in India).

Q: why is this underwear hand wash only ? isn't that a little extreme ?

Because if you wash them in the washer all the seams fall apart! Don’t buy this type. They have others that are wonderful!

Q: My husband bought these briefs; waistband is smooth on outside & rough inside:Chafes. Suggestions for different briefs?

First off, have your husband wear them inside-out! Thus the smooth is on the inside. I have found Fruit of the Loom to be more comfortable fit. I have switched to "hybrid" boxer-briefs which I find fit much better.
